Early Intervention Services

Call for an appointment at (603) 821-7788.

Early Intervention Services increase an individual’s awareness of their HIV status and, if needed, facilitate access to the HIV care system using HIV testing, referral services, health literacy/education and linkage to care as a bridge to medical care, medication access and treatment adherence.

In 2006 The Center Disease Control and Prevention (CDC) announced recommendation that everyone between the ages of 13-64 years old get tested at least once as part of routine healthcare and those with risk get tested more frequently.

At Harbor Care Health and Wellness Center, our EIS program offers:

  • Experienced Primary Care Providers for the entire family
  • Rapid HIV and routine testing
  • Flexible scheduling to meet your needs
  • Individual HIV risk reduction counseling and education
  • Pre-Exposure Prophylaxis Treatment (PrEP)
  • Post-exposure Prophylaxis (PEP)
  • Linkage to medical care and support
  • Healthy Sexual discussion, treatment, education, and provisions to keep you free of HIV and sexually transmitted infection (STI)
  • Post-exposure Prophylaxis (PEP)
  • Linkage to medical care and support

If you or someone you know might benefit from EIS services, call and schedule a primary care appointment today at (603) 821-7788.
