Harbor Care Health and Wellness Center welcomes you to your Patient Centered Medical Home. Caring about you is our most important job. Services are offered in all languages, including English and Spanish. Care is offered to anyone in need of health care by walk-in or appointment. Harbor Care Health and Wellness Center offers primary, preventive and behavioral health care services to children and adults who are homeless or at risk of homelessness, uninsured, under-insured, or low income. We also offer a sliding scale fee for self-pay patients, and accept Medicaid and Medicare, along with most insurances.

Who is on the patient-centered medical home care team?
Your primary care provider leads your care team, which may include specialized doctors. nurses, health educators, and other health care professionals such as pharmacists or physical therapists. Our team acts as “coaches” who help you get healthy, stay healthy, and get the care and services that are right for you.

To make a same day appointment for a sick visit, please call as early in the day as possible.
Plan to arrive at least 15 minutes prior to your scheduled appointment to complete necessary paperwork.

Cancellations and Late Policy
Please call 24 hours in advance to cancel an appointment. You may be asked to reschedule your appointment if you arrive more than 10 minutes after your scheduled appointment time.
In the event of a life-threatening emergency, call 911 or go directly to the nearest Emergency Department.

Please request prescription refills at the time of your office visit. If you need to refill a prescription between office visits, please call our Prescription Refill line (603)-821-7788
Depending on your family income some patients will be requested to make a co-payment at the time of your appointment for the services you receive. Your ability to pay will not affect the care you receive. Health services will be provided at no charge, or at a reduced charge, to persons unable to pay for services. In addition, persons will be charged for services to the extent that payment will be made by a third party authorized or under legal obligation to pay the charges.

Elderly Assistance – The Care Givers, Inc works with the disabled and elderly in Greater Nashua and Manchester, providing support services and transportation free of charge to those in need.
Prescription Assistance – Prescription Assistance program is a community benefit to help uninsured or underinsured patients receive medications through pharmaceutical companies.
Smoking Cessation – The New Hampshire Tobacco Helpline www.trytostopNH.org and National Cancer Institute www.Smokefree.gov can help you quit tobacco. Both resources have trained counselors who can help.
Substance Use Services – Keystone Hall provides comprehensive residential and outpatient substance use disorder treatment, prevention, and recovery supports to New Hampshire community members, including men, women, adolescents, and families. As a member of the Partnership for Successful Living, www.Keystonehall.org offers a unique, holistic approach to care that results in better outcomes for our clients and the community.
Skilled In-home Nursing – Healthy at Home provides a full range of in-home services to help New Hampshire residents remain independent in their own homes for as long as possible. Services include: skilled nursing, physical therapy, occupational therapy, home heath aides, personal care aides, companions, homemakers, respite care, transportation services, and seasonal yard clean-up. www.healthyathome.org
HIV/AIDS Treatment – The Southern New Hampshire HIV/AIDS Task Force provides affordable housing, case management, and supportive services including access to medical care and medications to individuals and families who are living with HIV/AIDS.
