Harbor Care Health and Wellness Center

Harbor Care Health and Wellness Center is working toward recognition as a Patient-Centered Medical Home where a team of health professionals work together to provide all of your health care needs. We use technology such as electronic medical records to communicate and coordinate your care and provide the best possible outcome for you.

Our medical home coordinates your care, which may include specialized doctors, nurses, behavioral health providers, health educators and other health care professionals.

Services are offered in all languages, including English and Spanish. Harbor Care Health and Wellness Center offers primary, preventive and behavioral health care services to children and adults who are homeless or at risk of homelessness,  uninsured, under-insured, or low income.  We also offer a sliding scale fee for self-pay patients, and accept Medicaid and Medicare, along with most insurances.

Harbor Care is available to patients 24/7. If you have an emergency, call 911. Otherwise, if you feel sick during the hours we are closed, our nurse triage at 603-821-7788 will be available to help you.

Insurance & Billing

What if I don't have insurance? Can I get assistance in finding quality, affordable health insurance coverage?

How will I be billed?
Depending on your family income some patients will be requested to make a co-payment at the time of your appointment for the services you receive.  Your ability to pay will not affect the care you receive. Health services will be provided at no charge, or at a reduced charge, to persons unable to pay for services. In addition, persons will be charged for services to the extent that payment will be made by a third party authorized or under legal obligation to pay the charges.
What if I cannot afford to pay?
This practice serves all patients regardless of inability to pay.Discounts for essential services are offered depending upon family size and income. You may apply for a discount at the front desk.

Medical Team

Who will I see at my appointment? Who is on my care team?
Your primary care provider leads your care team, which may include specialized doctors. nurses, health educators, and other health care professionals such as pharmacists or physical therapists.  Our team acts as “coaches” who help you get healthy, stay healthy, and get the care and services that are right for you.

Attending your appointment

Can I make an appointment for the same day?
We are often able to accommodate requests for same-day appointment. To make a same day appointment for a sick visit, please call as early in the day as possible.

In the event of a life-threatening emergency, call 911 or go directly to the nearest Emergency Department.

When should I arrive for my appointment?
Plan to arrive at least 15 minutes prior to your scheduled appointment to complete necessary paperwork.

What are your cancellation policies?
Please call 24 hours in advance to cancel an appointment.  You may be asked to reschedule your appointment if you arrive more than 10 minutes after your scheduled appointment time.

Prescription Refills

How can I get a refill of my prescription?
Please request prescription refills at the time of your office visit.  If you need to refill a prescription between office visits, please call our Prescription Refill line (603)-821-7788


Harbor Care Health and Wellness Center is supported through federal and state funding, as well as, donations. We are so grateful to our major supporter, the US Department of Health and Human Services, Health Resources Services Administration. Special thanks also go to the City of Nashua and the NH Department of Health and Human Services, Division of Public Health Services.

This health center receives HHS funding under 42 U.S.C.254b, and deemed Public Health Service employee status under 42 U.S.C.233(g)-(n) by the Federal Public Health Service (PHS) with respect to certain health or related claims, including medical malpractice claims, for itself and its covered individuals.
